Job summary

VNS Health in New York City seeks a Registered Nurse to join our interdisciplinary care team within a NORC or Senior Living community. You will coordinate health and social services to help residents age in place, perform clinical assessments, and advocate for residents across healthcare systems.

Responsibilities include health-focused outreach, referrals, direct patient care coordination with physicians, and documenting nursing interventions.

Qualifications

• License and current registration to practice as a Registered Professional Nurse in New York State required
• CPR/BLS Certification required
• Two years of LHCSA/CHHA RN experience preferred and working with aging population

Responsibilities

• Collaborates with Site staff to engage residents in self-care management health strategies.
• Provides health-focused outreach to seniors who may benefit from supportive services.
• Promote health activities such as health screening, assessments, linkages, and presentations.
• Makes telephone calls and home visits to assess needs and support community living.
• Makes referrals to health services based on assessed needs.
• Provides Health Care Advocacy by liaising between healthcare systems during transitions.
• Assist residents needing direct patient care to connect with physicians and services.
• Documents all nursing interventions as required by site location.
• Participates in special projects and other duties as assigned.
