Family & Children’s Services in the OKC/Norman area seeks an RN Care Manager to deliver integrated nursing and case management for individuals with behavioral health, substance use, and medical needs. You will assess, educate, coordinate care with interdisciplinary teams, and advocate for clients to support recovery and improved health outcomes.

Qualifications

  • ADN required with minimum 36 months of professional nursing experience in behavioral health and/or substance use disorder treatment.
  • BSN preferred with minimum 6 months of professional nursing experience in behavioral health and/or substance use disorder treatment.

Responsibilities

  • Provide professional nursing care to individuals with serious mental illness, substance use disorders, and co-occurring conditions.
  • Provide case management services.
  • Conduct and document nursing assessments.
  • Administer and monitor prescribed medications in accordance with provider orders.
  • Support health promotion and disease management through nursing education related to behavioral health conditions and medications.
  • Coordinate nursing care with treatment teams to promote continuity of care and safe transitions.
  • Provide nursing follow up after hospital or ED utilization.
  • Monitor engagement and adherence to treatment plans and clinical guidelines.
  • Apply evidence-based and trauma-informed nursing principles.
  • Provide nursing crisis intervention.
  • Educate clients and families with consent about care coordination and treatment plans.
  • Accompany clients to medical or behavioral health appointments as needed.
  • Assist clients in accessing community resources for stabilization and wellness.
  • Maintain documentation in compliance with policy and regulations.
  • Participate in quality improvement, utilization review, and outcome monitoring.

Job description

This position is located in the OKC/Norman area.

The RN Care Manager delivers integrated nursing and case management services for individuals with behavioral health, substance use, and medical needs. This role provides assessments, direct nursing care, care coordination, and health education while collaborating with interdisciplinary teams and community providers to support continuity of care, recovery, and improved health outcomes.

Position Specific Duties & Responsibilities
  • Provide professional nursing care to individuals with serious mental illness, substance use disorders, and co occurring conditions.
  • Provide case management services.
  • Conduct and document nursing assessments.
  • Administer and monitor prescribed medications in accordance with provider orders.
  • Support health promotion and disease management by providing nursing education related to behavioral health conditions, physical health comorbidities, medications, relapse prevention, and wellness activities within the RN scope of practice.
  • Coordinate nursing care and services with treatment team to promote continuity of care and safe treatment transitions.
  • Provide nursing follow up after hospital or emergency department utilization.
  • Monitor client engagement, service utilization, and adherence to treatment plans and clinical guidelines, identifying changes in condition or risk and escalating per protocol.
  • Apply evidence based and trauma informed nursing principles in the care of individuals.
  • Provide nursing crisis intervention.
  • Engage clients and, with appropriate consent, families or designated supports in nursing education, care coordination, and reinforcement of provider directed treatment plans.
  • Accompany clients to medical or behavioral health appointments as needed to provide nursing support, education, and advocacy within the RN role.
  • Assist clients in accessing community based resources to support stabilization, recovery, and whole person wellness.
  • Maintain documentation of nursing assessments, interventions, medication administration, client responses, and coordination activities in compliance with organizational policy, OBN standards, and applicable regulations.
  • Participate in quality improvement, utilization review, and outcome monitoring activities, to support program effectiveness and compliance.
Qualifications

EDUCATION:

  • Bachelor's Degree in Nursing (BSN) Preferred
  • Associate's Degree in Nursing (ADN) Required.
Experience
  • ADN prepared nurses must have a minimum of thirty six (36) months of professional nursing experience in behavioral health and/or substance use disorder treatment.
  • BSN prepared nurses preferred to have a minimum of six (6) months of professional nursing experience in behavioral health and/or substance use disorder treatment.

Certifications/Licenses
  • Must be a licensed Registered Nurse in the State of Oklahoma
  • ODMHSAS Case Manager II Certification, completed within 60 days of eligibility.
  • Must possess a valid Driver License and satisfactory driving record to use agency and/or personal automobile to travel to locations other than primary office and/or for the transportation of clients.

This job is classified as a “safety-sensitive” position as defined by the Oklahoma Medical Marijuana and Patient Protection Act. Due to the “safety-sensitive” classification, an employee in this position would be subject to drug and alcohol testing, including random testing.
