Qualifications

• RN license in Michigan required.
• CPR certification (BCLS) required.
• Valid Michigan driver’s license and auto insurance.
• Experience in care coordination or PCMH a plus.

Responsibilities

• Perform comprehensive assessment of patients’ physical, mental, and psychosocial needs.
• Develop and implement care plans to prevent disease progression and reduce hospital use.
• Coordinate specialty care and follow up on test results.
• Support PCMH initiatives and participate in team-based care.
• Document progress notes accurately in the EHR and communicate status to providers.
