This role ensures personalized patient care by developing care plans and collaborating with team members for chronic disease management and preventive health. The ideal candidate will have a Bachelor's degree in Nursing and preferably two years of clinical experience.

Qualifications

• Minimum of two years’ clinical experience preferred.
• Currently holds an unencumbered RN license.
• Basic Life Support (BLS) required within six months.

Responsibilities

• Utilize the nursing process to provide individualized nursing care.
• Develop and provide individualized patient education.
• Coordinate patient care across the continuum.
