Job summary

TEEMA seeks an experienced Registered Nurse Care Manager to coordinate home health services across clinics, centers, and patient homes. You will assess needs, develop care plans, and ensure smooth transitions between care settings while maintaining patient-centered focus.

The role involves a 24/7 on-call rotation, collaboration with providers, and ongoing education for patients, families, and teammates. Travel to patient homes may be required.

Qualifications

• Active, unrestricted RN license in the applicable state.
• BSN preferred.
• BLS certification within 90 days of employment.
• Minimum one year of RN experience.
• Reliable transportation and valid driver’s license.
• Ability to meet health/immunization requirements.
• Background check including criminal history, license verification and drug screening.

Responsibilities

• Deliver compassionate nursing care in clinics, centers, contracted facilities, and patients' homes.
• Participate in a 24/7 on-call rotation for triage and support.
• Assess, plan, and coordinate home care services per care plans.
• Monitor acute and chronic conditions; support transitions of care.
• Coordinate with providers; obtain reports, labs, and documentation.
• Perform monthly medication reconciliation and address discrepancies.
• Educate patients and families on self-care and safety practices.
• Review and revise care goals with the team.
• Delegate tasks to CNAs/Home Care Aides and ensure competency.
• Mentor team members and uphold confidentiality and infection control.
