Job summary

Geisinger is seeking a Community-Based RN Care Manager to join our at-home care team in Mifflin County, PA. You will advocate for patients, develop and coordinate personalized care plans with a multidisciplinary team, and guide patients from admission through discharge.

The role requires a minimum of 3 years of nursing experience, with preferred background in ED, ICU, or case management, and is based on a Monday–Friday schedule with a late-day option and rotating weekends/holidays.

Qualifications

• RN with minimum 3 years of nursing experience.
• Experience in ED/ICU and/or case management preferred.
• Community-based nursing home visits in Mifflin County, PA.

Responsibilities

• Serve as a patient advocate for navigating healthcare needs.
• Develop personalized care plans and coordinate resources with physicians and interdisciplinary teams.
• Monitor patient progress and ensure adherence to care plans.
• Coordinate discharge planning and community services for patients.
• Collaborate with physicians, nurses, and other team members to manage care from admission to discharge.
