Job summary

Molina Healthcare is seeking an RN Care Manager to support care coordination for LTSS members in New York. You will collaborate with a multidisciplinary team to develop and monitor care plans, address barriers, and ensure services are delivered timely and effectively.

Responsibilities include comprehensive member assessments, waiver enrollment management, and coordinating with providers, caregivers, and the member support network. Travel up to 40% may be required depending on state contracts.

Qualifications

• At least 2 years in health care with 1 year in care management or LTSS.
• RN licensed and active in state of practice.
• Proactive, detail-oriented and able to work with diverse populations.
• Ability to travel 25-40% locally as required by state contracts.

Responsibilities

• Completes comprehensive member assessments within regulated timelines.
• Facilitates waiver enrollment/disenrollment processes.
• Develops and implements care plans with members and care network.
• Monitors care plan effectiveness and documents interventions.
• Promotes integrated services across LTSS and behavioral health.
• Assesses medical necessity and authorizes waiver services.
• Coordinates ICT meetings for approval/denial of services.
• Educates and motivates members during contacts.
• Addresses psycho/social, financial, and medical barriers to care.
