Job summary

MLK Community Healthcare in Los Angeles, CA is seeking a Registered Nurse Care Manager to coordinate care for patients enrolled in the Enhanced Care Management program. The RN Coordinator will lead care plans, participate in Interdisciplinary Care Team meetings, and support transitions to primary care, behavioral health, or community services.

The role emphasizes nursing assessments, medication management, social needs, and patient engagement, with on-site responsibilities and collaboration

Qualifications

• Minimum of one (1) year of case/care management experience required.
• Demonstrated ability to build relationships with peers and leaders.
• Current Basic Life Support (BLS) certification from the American Heart Association.
• Current Advanced Cardiac Life Support (ACLS) from the American Heart Association, preferred.
• Must complete annual Workplace Violence Prevention Program/Certificate within 30 days of hire.
• Valid unrestricted CA Driver’s License.

Responsibilities

• Provides Clinical Oversight of ECM Lead Care Managers and develops patient-centered care plans.
• Ensures ECM members receive needed services and measures progress toward goals.
• Oversees quality measures across access, clinical, and experiential benchmarks.
• Performs nursing assessments, medication management and adherence review.
• Conducts home visits and community outreach to assist vulnerable populations.
• Coordinates care with PCP, behavioral health providers and specialists.
