Qualifications

• Graduation from an accredited school of nursing.
• Oregon Registered Nurse License at hire.
• 3 years clinical RN experience.
• Preferred: Bachelor’s Degree in Nursing.
• 2 years Home health, skilled nursing facility and/or community health nursing.
• 2 years Managed care experience and/or utilization management experience.
• 2 years Care/Case Management experience.

Responsibilities

• Provide care management and care coordination for at‑risk patients.
• Apply population-based chronic care management approaches and clinical protocols.
• Be a core member of patient-centered medical home (PCMH) teams led by physicians/providers.

Education

Bachelor’s Degree in Nursing

Oregon Registered Nurse License

3 years Clinical experience as an RN
