Job summary

Horizon Blue Cross Blue Shield of New Jersey is seeking an experienced RN to ensure appropriate levels of care and coordinate transitions in the continuum of care. You will assess clinical needs, evaluate services, and collaborate with patients, families, physicians, and hospitals to optimize outcomes.

The role offers a Monday–Friday schedule with 8- or 10-hour days in Hopewell, NJ, with a pay range of $79,100–$105,945 annually and comprehensive health benefits.

Qualifications

• RN licensure in NJ or Compact license.
• Minimum 2 years clinical experience.
• Minimum 2 years’ payer experience.

Responsibilities

• Assess clinical needs against guidelines to determine care levels.
• Evaluate necessity and efficiency of services and procedures.
• Coordinate and implement care plans for members.
• Coordinate with patients, families, physicians, and hospitals.
• Document accurately per practice standards and policies.
• Mentor/trainer role for new RNs and staff.
