Qualifications

• Graduate of an accredited School of Nursing or Social Work, or equivalent case management experience with any bachelor's degree.
• Bachelor’s in Nursing or Master’s in Social Work preferred.
• Minimum 2 years of applicable clinical experience; experience with utilization management and discharge planning preferred.

Responsibilities

• Coordinates care for assigned patient populations and promotes appropriate resource use.
• Leads the Multidisciplinary Care Team to optimize clinical and financial outcomes.
• Develops goal-directed care plans based on diagnoses and patient needs.
• Discharge planning: communicates plans timely with patients and providers.
• Ensures rights and discharge appeal processes are explained during denials.
