Job summary

You will review assessments, support discharge planning, verify admission status, educate patients on options, document in medical records, and serve high‑risk populations including homeless and mental health patients.

Qualifications

• BSN required.
• Registered Nurse (RN) license required.
• One year of experience in an acute health care setting preferred.
• Fulfills mandatory stroke education requirements per certification agency.

Responsibilities

• Coordinate patients’ needs through the continuum of care from pre-admission to post-discharge planning.
• Collaborate with physicians, nurses, clinical staff, and community agencies to identify and arrange appropriate care.
• Review clinician assessments and patient financial, family, and psychosocial support to develop comprehensive discharge plans.
• Educate patients and families on healthcare options and connect them with resources.
• Document pertinent patient issues, contacts and plans in medical records; act as mandated reporter for elder, child, and spousal abuse.
• Provide Care Management support to high-risk populations such as homeless and mental health patients; assist geriatric patients as needed.
