Job summary

Qualifications

• RN licensure in the state of practice is required.
• BSN degree preferred.
• Case management experience or certification preferred.
• Experience coordinating care across disciplines preferred.

Responsibilities

• Leads coordination of patient care with the care team.
• Monitors appropriateness and timeliness of care.
• Ensures care plan aligns with clinical course and discharge planning.
• Coordinates transfer of patients to tertiary centers as needed.
• Oversees data collection for regulatory compliance.
• Performs other duties as assigned.
