Job summary

Martin Luther King, Jr. Community Hospital seeks an experienced Case Manager to support physicians and the interdisciplinary team in optimizing patient outcomes and satisfaction while controlling costs during hospitalization and transitions of care.

The role requires comprehensive assessments, coordination of utilization management, discharge planning, and timely communication with payors, providers, and patients to ensure safe, efficient care progression across the continuum.

Qualifications

• Bachelor of Science in Nursing preferred.
• Minimum 1–3 years of hospital case management/coordination experience.
• Able to navigate outside provider networks (Health Plans, IPA, FQHCs).
• Bilingual in Spanish preferred; basic computer skills required.

Responsibilities

• Completes comprehensive assessments to identify interventions for psycho-social, cultural, spiritual, and physical plans of care.
• Coordinates progression of care to meet patient needs and optimize length of stay and resource use.
• Interacts with physicians to discuss appropriateness of admissions and documentation for medical necessity.
• Collaborates with interdisciplinary team to develop and adjust transition plans.
• Assists with discharge planning, transfers, and home health/DME coordination.
• Maintains communication with Patient Financial Services and HIM for authorization and status.
