Job summary

Point32Health in Canton, MA is seeking an RN Care Manager to ensure timely care management across the continuum, including transitions of care, care coordination, and complex case management.

The role requires a Bachelor’s degree in nursing, 5 years’ clinical experience, and an unrestricted RN license, with national Case Management certification preferred. Responsibilities include outreach, assessments, care plans, documentation, and collaboration with interdisciplinary teams.

• Collaborate with the facility care team for safe transitions to the next level of care.
• Liaise with the interdisciplinary care team to improve member outcomes.
• Attend and present high-risk members at interdisciplinary rounds.
• Maintain professional growth through self-directed learning activities.
