Qualifications

• Active NY RN license required.
• BSN preferred; Bachelor's degree required.
• Minimum 5 years of clinical RN experience.
• 2–3 years of CHHA, Lombardi, MLTC or MAP program experience preferred.
• Care management and care coordination experience required.
• Utilization review and discharge planning experience preferred.
• Leadership experience preferred.
• Computer literacy required.
• PRI and Screen certification required within 3 months.
