Job summary

Jobtailor in Colorado is seeking an experienced Registered Nurse to coordinate care for patients with chronic or complex conditions in a collaborative healthcare setting. You will work with physicians, interdisciplinary teams, patients and families to implement and monitor care plans across the continuum.

The ideal candidate has a BSN, an active RN license, and at least two years in primary or ambulatory care with care management experience. Certification such as CCM is preferred.

Qualifications

• Current RN license in state of practice.
• BSN from an accredited institution (degree verification required).
• Two (2) years of nursing experience, preferably in primary care or ambulatory care settings.
• Demonstrated experience in care management.
• CCM or other relevant certification preferred.
• Knowledge of value-based care models and principles.
• Familiarity with patient-related terminology and processes.

Responsibilities

• Coordinate care across physicians, interdisciplinary teams, patients and families.
• Perform comprehensive care continuum assessments and planning.
• Educate and coach patients and caregivers on treatment options.
• Support goal-setting and development of self-care skills.
• Facilitate communication within the healthcare team.

• Coordinates and manages the care of patients with chronic or complex conditions
• Works collaboratively with physicians, interdisciplinary teams, individual patients, and families
• Performs comprehensive care continuum process including assessment, planning, implementation, coordination, monitoring, and evaluation
• Advocates for, educates, and coaches patients, families, and caregivers on treatment options
• Supports goal-setting and development of self-care skills
• Facilitates communication and coordination among members of the healthcare team

Requirements

• Current Registered Nurse (RN) license in state of practice
• Bachelor of Science in Nursing (BSN) from an accredited institution (degree verification required)
• Demonstrated clinical nursing experience, and familiarity with patient-related terminology and processes
• Demonstrated experience in care management
• Certified Case Manager (CCM) or other relevant certification preferred
• Two (2) years of experience in nursing, preferably in primary care or ambulatory care settings
• One (1) year of experience in care management
• Experience in chronic disease management
• Knowledge of value-based care models and principles
