Job summary

Fairview Hospital / Cleveland Clinic seeks an RN Hospital Care Manager I to deliver comprehensive care management across inpatient and outpatient settings. You will conduct screenings, assessments, and evaluations, and develop patient-centered plans in collaboration with patients, families, and providers.

You will lead care transitions, support self-management, and educate care teams while tracking metrics and ensuring regulatory compliance in a high-quality care environment.

Qualifications

  • Current RN license in the practice state.
  • BSN; must obtain BSN within four years if hired without one.
  • Clinical nursing experience in inpatient hospital settings with knowledge of hospital processes.
  • Understanding of disease management, barriers to care, and length of stay.
  • Skills in assessment, care planning, motivational interviewing, critical thinking, communication, time management, and patient education.
  • Preferred: case management certification, discharge planning, utilization review, or value-based care; basic computer proficiency.

Job description

Role Overview

The RN Hospital Care Manager I provides comprehensive care management for designated patients, utilizing clinical expertise to conduct screenings, assessments, and evaluations. They develop and implement patient-centered care plans, collaborating with patients, families, healthcare providers, payers, and community resources to ensure efficient, high-quality care across inpatient and outpatient settings.


Key Responsibilities
