Job summary

Standing Stone Care & Rehabilitation Center in Tennessee is seeking an experienced RN to join our dedicated team delivering high-quality nursing care to residents. You will provide direct nursing care, administer medications, supervise nursing assistants, and coordinate care with the interdisciplinary team.

We value strong clinical judgment, documentation accuracy, and the ability to work independently while guiding staff through daily shifts.

Qualifications

• Active RN license in the state of practice.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role.
• Knowledge of Federal and State long-term care regulations.
• Strong clinical observation, medication administration, and documentation skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing.
