• Current and active California RN license.
• AHA BLS certification.

Responsibilities

• Oversee discharge planning assessment and update plans as needed.
• Develop multidisciplinary discharge plans with health team and post-acute providers.
• Oversee and evaluate discharge plan implementation.
• Coordinate with healthcare teams and post-acute services for timely transitions.
• Deliver patient notifications and related documentation.
• Participate in performance improvement initiatives.
