Job summary

CentraState Medical Center in Freehold, NJ is seeking a Care Coordinator to manage patient care from pre-admission through discharge, coordinating resources to ensure high-quality service and favorable financial outcomes. The role requires a Bachelor's in Nursing, at least 2 years of Acute Care RN experience, and an active NJ RN license.

Knowledge of discharge planning, utilization review, and quality improvement is preferred.

Qualifications

• Utilizes effective communication with patients, family, physicians, nurses, staff and others to achieve outcomes.
• Plans, organizes and prioritizes work to impact clinical, financial and service outcomes.
• Facilitates post-hospitalization discharge needs and coordinates with social worker.
• Maintains communications with services to ensure quality care and timely discharge planning.
• Collects and documents accurate data.
• Assesses and identifies patients needing Case Management in ED.
• Collaborates with team for bed assignment and admission through Interqual criteria.
• Develops initial discharge plans for ED admitted and observation patients.
• Offers alternative care settings when needed.
• Participates in performance improvement in ED for optimal outcomes.

CentraState Medical Center has an employment opportunity available for a Care Coordinator. This role facilitates patient care from pre-admission through discharge by managing, coordinating, and monitoring resources to ensure optimal financial outcomes, efficient resource utilization, and high-quality service.

Responsibilities

• Provides education to the patient/family regarding the disease processes and planned treatment/intervention as it relates to discharge plans.
• Reviews hospital records to determine medical necessity for admission and continued stay according to specific criteria and regulations.
• Utilizes effective communication strategy with patients, family, physicians, nurses, staff members and other professionals to achieve desirable or acceptable patient outcomes and a perception of satisfaction from the individuals involved.
• Plans, organizes and prioritizes work to obtain the greatest impact on clinical, financial and service outcomes.
• Facilitates post-hospitalization discharge needs. Coordinates services in collaboration with social worker.
• Maintains communications with all relevant services to ensure quality care and timely discharge planning.
• Collects and documents accurate data within established time frame.
• Assesses and identifies patients requiring Case Management intervention in the Emergency Department.
• Collaborates with other team members to assure appropriate bed assignment and admission through application of Interqual criteria.
• Develops an initial discharge plan for selected patients in both Emergency Department admitted patients and observation patients.
• Offers alternative care settings when criteria indicate a need or additional clinical documentation is required.
• Participates in performance improvement activities in Emergency Department in an effort to provide optimal outcomes and patient and staff satisfaction.
• Serves as a resource to the Emergency Department and hospital as it relates to System/community resources and nursing care facilities for OBS, ED, and admitted patients.

• Works with Emergency Department and attending physician to facilitate use of admission order sets.

Qualifications

• Bachelor's Degree in Nursing required.
• A minimum of 2 years of Acute Care RN experience required.
• Current NJ RN License required.
• Knowledge of utilization, quality improvement, case management, or discharge planning required.
• Computer literacy preferred.
• Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.

Compensation Range: $48.00 - $81.60 hourly

The compensation above reflects the established range from CentraState Healthcare System (CSHS) for this position at the time the job was posted. CSHS considers many factors to determine compensation, including education, experience, skills, licenses, certification, and training. As such, team member compensation may fall outside this range.

Additionally, the compensation range reflects base salary and does not include extra shift rates or incentives tied to quality, productivity, etc., as applicable.
