The RN screens admissions, coordinates with an interdisciplinary team, documents in EMR, conducts TB surveillance, manages medications, provides education, and ensures safety throughout care transitions. A strong clinical foundation and team collaboration are essential.

Qualifikationen

• Maintain current NYS Registered Nurse license.
• Bachelor’s degree preferred.
• Minimum 1 year experience with substance use disorders and co-occurring mental health disorders.
• Maintain CPR Certification and Narcan training.
• NYS driver’s license preferred.

Aufgaben

• Screen admissions via telephone or walk-ins to determine level of care and document Initial Determination of Services.
• Refer individuals to other services if not eligible or beds unavailable; document non-admission reasons.
• Process intake with the interdisciplinary team and complete forms in EMR (consents, evaluations).
• Ensure orientation to unit, confidentiality, safety policies, and infection control are followed.
• Collect health data, perform health screening, and document nursing initial assessment; obtain records when needed.
• Administer and supervise medications, provide education, and document Medication Self-Administration Records.
• Follow orders from medical/psychiatric practitioners, including telephone orders.
• Monitor withdrawal signs and provide general nursing care; facilitate transitions as needed.
• Provide rescue care (CPR, Narcan) if necessary and document interventions.
• Document rounds, discharge plans, and patient education with the team.
