Job summary

HealthFirst Bluegrass is seeking a mission-focused RN leader to expand access to behavioral health care, keep care compassionate, and serve as Behavioral Health Services Manager to support patients with complex needs.

You will coordinate RN-level clinical care, triage, care planning, medication management, and patient education while partnering with clinical, operational, quality, finance, IT, and case management teams to improve workflows and patient outcomes.

Qualifications

• Current Kentucky RN license and CPR certification required.
• Nursing degree required; bachelor’s degree or equivalent combination of education and experience required.
• Three to five years of behavioral health and healthcare operations experience preferred.
• Experience in integrated care, community health, FQHC operations, outreach, grants, project management, program development, or quality improvement preferred.

Responsibilities

• Provide RN clinical coordination for high-risk patients.
• Support care planning, triage, medication management, and patient education.
• Coordinate with clinical, operational, quality, finance, IT, patient access, and case management teams to improve workflows, outcomes, and the patient and staff experience.
• Track deliverables, outcomes, and compliance for grant-funded programs.
• Lead with HealthFirst’s CARES values—compassion, accountability, respect, empathy, continuous improvement, and service excellence—while strengthening behavioral health services.
