Qualifications

Responsibilities

• Assess, plan, implement, and evaluate individualized patient care plans.
• Monitor patients' physical and behavioral health status and respond to changes.
• Administer medications and treatments per orders and policies.
• Collaborate with psychiatrists, physicians, therapists, social workers, and interdisciplinary team members.
• Maintain accurate documentation in the EMR.
• Support a safe, therapeutic environment with de-escalation and crisis intervention.
• Participate in treatment meetings and discharge planning.
• Educate patients and families about treatment plans and goals.
• Ensure compliance with hospital policies and patient safety initiatives.
• Advocate for patients and promote recovery-focused care.
