Job summary

Sycamore Springs is seeking an RN, Assessment Specialist/Intake Specialist in Lafayette, IN, with a $5,000 sign-on bonus. The role focuses on rapid response to service requests, initial assessment, referral, and determining the appropriate care level, followed by timely admissions and follow-up.

Requires nursing or social work/counseling education and active clinical licensure. Responsibilities include documenting patient status, coordinating with MDs for LOC decisions, and ensuring accurate

Qualifications

• Associate's Degree in Nursing or master's in social work/counseling required.
• CPR and de-escalation certification required or obtained within 30 days of hire.
• Current unencumbered RN, clinical social work, or counseling license per state guidelines.

Responsibilities

• Facilitates response to service requests, initial assessment, and referral process.
• Determines level of care and admission into services with follow-up activity.
• Performs initial evaluation and documents symptoms, functioning, and needs.
• Provides information to MD for LOC determination and supports patients, physicians, visitors.
• Maintains preadmission and admission paperwork and precertification with insurers.
