Responsibilities

• Acts as patient advocate in ambulatory settings and navigates cross-setting care.
• Addresses care coordination needs for high and moderate risk patients.
• Reviews patient records to identify patients needing care coordination.
• Assesses and addresses clinical, psychosocial, or financial barriers to care.
• Consults with multidisciplinary team to develop and evaluate patient-focused care.
• Provides triage care using in-depth clinical knowledge in area of expertise.
• Educates patients and families about medical processes, treatments, and health management.
• Ensures continuity of care post-discharge, manages referrals and transitions, and coordinates community resources.
