Job summary

Personal Touch Home Care is seeking compassionate Registered Nurses to provide high-quality in-home care in the Kentucky region. You will assess patients, develop and execute individualized care plans, administer medications, and coordinate with a multidisciplinary team to ensure comprehensive care at the patient’s home.

The role includes wound care, patient education, and ongoing evaluation of outcomes, with emphasis on autonomy, safety, and family involvement in care planning.

Qualifications

• Bachelor’s degree in Nursing or equivalent.
• Licensed RN in Kentucky in good standing.
• 1 year of community health/home health nursing experience.
• Knowledge of nursing practices, regulations, and guidelines for home care.

Responsibilities

• Assess and plan patient care with individualized plans.
• Administer medications and monitor for adverse effects.
• Provide wound care and dressing changes.
• Educate patients and families about conditions and self-care.
• Monitor patient progress and adjust care plans as needed.
• Coordinate care with physicians, therapists, and aides.
• Maintain accurate medical records and documentation.
• Provide emotional support and advocate for patients and families.

Education

Bachelor’s degree in Nursing

Graduate of an approved school of nursing

Licensed Registered Nurse in Kentucky

JOB SUMMARY

• Assessment and Care Planning:Conduct initial and ongoing assessments of patients' health status. Develop and implement individualized care plans based on these assessments to meet the patient's needs.
• Medication Administration:Administer medications and treatments as prescribed by the physician. Monitor and assess the patient for any adverse reactions or side effects.
• Wound Care:Provide wound care management, including dressing changes, infection control, and monitoring healing progress.
• Patient Education:Educate patients and their families about their medical conditions, medications, and treatment plans. Teach them techniques for self-care and management of their health at home.
• Monitoring and Evaluation:Monitor patients' progress and condition regularly. Evaluate the effectiveness of the care plan and make necessary adjustments based on patient outcomes.
• Collaboration:Coordinate care with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to ensure comprehensive and integrated care for the patient.
• Documentation: Maintain accurate and up-to-date medical records and documentation of nursing assessments, interventions, and patient responses.
• Support and Advocacy:Provide emotional support and advocacy for patients and their families. Ensure patients' rights and preferences are respected throughout their care.

QUALIFICATIONS

• Graduate of an approved school of nursing or Bachelor’s degree of Nursing from an accredited college/university
• Currently licensed, in good standing, to practice in Kentucky as a Registered Nurse.
• Minimum 1 year community health nursing experience or equivalent professional experience.
• Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS.
• Complies with accepted professional standards and practice.
