Job summary

Personal Touch Home Care in Kentucky is seeking compassionate Registered Nurses to provide in-home assessments, medication administration, wound care, and patient education for patients in their own homes. This role emphasizes collaboration with physicians, therapists, social workers, and home health aides to ensure comprehensive care and patient well-being.

Qualifications

  • Graduate of an approved school of nursing or Bachelor’s degree of Nursing from an accredited college/university.
  • Currently licensed, in good standing, to practice in Kentucky as a Registered Nurse.
  • Minimum 1 year community health nursing experience or equivalent professional experience.
  • Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS.

Responsibilities

  • Assessment and Care Planning: Conduct initial and ongoing assessments of patients' health status. Develop and implement individualized care plans based on these assessments to meet the patient's needs.
  • Medication Administration: Administer medications and treatments as prescribed by the physician. Monitor and assess the patient for any adverse reactions or side effects.
  • Wound Care: Provide wound care management, including dressing changes, infection control, and monitoring healing progress.
  • Patient Education: Educate patients and their families about their medical conditions, medications, and treatment plans. Teach them techniques for self-care and management of their health at home.
  • Monitoring and Evaluation: Monitor patients' progress and condition regularly. Evaluate the effectiveness of the care plan and make necessary adjustments based on patient outcomes.
  • Collaboration: Coordinate care with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to ensure comprehensive and integrated care for the patient.
  • Documentation: Maintain accurate and up-to-date medical records and documentation of nursing assessments, interventions, and patient responses.
  • Support and Advocacy: Provide emotional support and advocacy for patients and their families. Ensure patients' rights and preferences are respected throughout their care.
