Job summary

Personal Touch Home Care is seeking compassionate Registered Nurses to provide in-home care in the Maysville area. You will assess patients, plan individualized care, administer medications, manage wounds, educate families, monitor progress, coordinate with the care team, and document outcomes.

Qualifications

  • Graduate of an approved school of nursing or Bachelor’s degree in Nursing from an accredited college/university.
  • Currently licensed, in good standing, to practice in Kentucky as a Registered Nurse.
  • Minimum 1 year community health nursing experience or equivalent professional experience.
  • Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS.
  • Complies with accepted professional standards and practice.

Responsibilities

  • Assessment and Care Planning: Conduct initial and ongoing assessments of patients' health status and develop individualized care plans.
  • Medication Administration: Administer medications and monitor for adverse reactions.
  • Wound Care: Provide wound care management including dressing changes and infection control.
  • Patient Education: Educate patients and families about conditions, medications and self-care techniques.
  • Monitoring and Evaluation: Track progress and adjust care plans as needed.
  • Collaboration: Coordinate with physicians, therapists and home health aides for integrated care.
  • Documentation: Maintain accurate medical records of nursing assessments and interventions.
  • Support and Advocacy: Provide emotional support and advocate for patient rights.

Job Summary
  • Assessment and Care Planning: Conduct initial and ongoing assessments of patients' health status. Develop and implement individualized care plans based on these assessments to meet the patient's needs.
  • Medication Administration: Administer medications and treatments as prescribed by the physician. Monitor and assess the patient for any adverse reactions or side effects.
  • Wound Care: Provide wound care management, including dressing changes, infection control, and monitoring healing progress.
  • Patient Education: Educate patients and their families about their medical conditions, medications, and treatment plans. Teach them techniques for self‑care and management of their health at home.
  • Monitoring and Evaluation: Monitor patients' progress and condition regularly. Evaluate the effectiveness of the care plan and make necessary adjustments based on patient outcomes.
  • Collaboration: Coordinate care with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to ensure comprehensive and integrated care for the patient.
  • Documentation: Maintain accurate and up‑to‑date medical records and documentation of nursing assessments, interventions, and patient responses.
  • Support and Advocacy: Provide emotional support and advocacy for patients and their families. Ensure patients' rights and preferences are respected throughout their care.
