Job Summary
To provide professional nursing care in accordance with the Nurse Practice Act, to collaborate with the physician's medical plan, and to execute the objectives and the policies of the hospital.
Accountabilities
- Assess, plan, implement, document, coordinate, and manage patient care according to department and hospital standards, giving consideration to age‑specific accommodations; utilize the nursing process for all patients assigned to her/his care; perform and document comprehensive admission assessments and timely shift assessments.
- Initiate a computerized plan of care and shift goal based on nursing assessment, the medical plan of care and patient needs, and maintain and revise it using evidence‑based practice.
- Provide patient care that demonstrates comprehensive knowledge of normal physiology, disease pathophysiology, appropriate nursing and medical interventions, and principles of growth and development over the lifespan.
- Document patient response to treatment, plan of care and shift goal; provide patient education based on individual healthcare needs on an ongoing basis; complete documentation including transcription of physician orders and computer entry without unapproved abbreviations; demonstrate ability to use the computer for automated order entry, data retrieval, nursing documentation, intradepartmental and interdepartmental communication.
- Evaluate, revise, and improve patient care using creative strategies, evidence‑based nursing, and consideration of age and population‑specific accommodations; appropriately assign all associates under her/his supervision according to skill level.
- Implement nursing/physician orders to achieve patient, nursing, and physician goals; demonstrate initiative and self‑direction in completing assigned responsibilities according to department needs.
- Analyze data and respond to changes in patient status by consulting appropriate healthcare team members and taking appropriate action in a timely manner.
- Perform selected therapeutic interventions and diagnostic procedures according to department standards, which include but are not limited to: point‑of‑care testing, initiation and maintenance of oxygen therapy, 12‑lead ECG, insertion and maintenance of nasogastric tubes (NGT), flushing and maintenance of central lines—including PICC lines, Groshong PICC, single, double and triple central lines, sub‑Q ports when accessed.
- Shift and 24‑hour chart check; use two patient identifiers before performing interventions and/or procedures; interpret rhythm strips; maintain vasoactive medications appropriate to telemetry; care for patients physically and psychologically with consideration of age, cultural, and spiritual issues.
- Perform with clinical proficiency in crises and medical emergencies.
- Communicate and collaborate with patients, physicians, family members and other health‑care disciplines to achieve excellence in patient care during hospitalization; utilize effective interpersonal skills; involve patient/significant other in the plan of care, serving as a patient advocate; consistently and continuously communicate pertinent information regarding changes in patient status and patient care with appropriate members of the healthcare team; use interpersonal communication techniques to give constructive feedback; effectively communicate pertinent patient/significant other information during handoffs according to department standards; demonstrate leadership skills in team building and interpersonal relationships to create a cohesive and productive environment; recognize interdepartmental relationships and interact effectively; follow staffing guidelines and respond to changes or needs within the Patient Services Team by cooperating with staff rotation, reassignment to other departments, and weekend/holiday scheduling.
- Promote patient safety through adherence to protocols, policies, procedures, evidence‑based guidelines and Joint Commission National Patient Safety Goals; maintain safety precautions while providing patient care—including infection control isolation, patient/bed positioning, side rails, restraint utilization, skin care, patient/specimen transport, physical environment, fall prevention, seizure precautions, and work injury prevention; consistently wear hand hygiene badge and maintain compliance at 70 % or above; maintain equipment in good working order, report equipment failure, and initiate corrective action; safely administer medications and treatments as ordered, using two patient identifiers and considering age‑specific effects; safely handle drugs and body fluids using standard precautions.
- Actively participate in and support departmental and organizational quality initiatives and goals; identify professional goals that reflect own strengths and weaknesses and seek assistance for improvement; attend and participate in meetings according to department standards; attend a minimum of four intershifts per year; support and participate in department, hospital, and ISLT evidence‑based care quality initiatives such as heart failure, acute MI, glycemic control, VTE prophylaxis, pressure ulcer prevention, rapid response team, and infection control initiatives.
- Maintain clinical competency according to unit‑specific standards; recognize legal responsibilities and functions within the scope of the PA Nurse Practice Act; demonstrate competency in maintaining specific technological skills required by job category and/or specialty area; maintain orthopedic competency and clinical training; acquire and maintain proficiency in the maintenance of IV lines and sites; complete annual mandatory requirements including CPR certification for healthcare providers, RAHE, specific departmental competencies, point of care testing, fit testing, ACLS certification within one year of hire or transfer to 4 Acute Care, NIHSS certification, and completion of unit‑based skills sessions yearly; perform duties in accordance with Penn Medicine and entity values, policies, and procedures; complete other duties as assigned to support the unit, department, entity, and health system organization.
Qualifications
- BSN required.
- Proof of completion of critical‑care course or completion of course within one year of hire.
Licenses, Registrations, and Certifications
- Certified in BLS for healthcare providers; ACLS required.
- Current licensure in the Commonwealth of Pennsylvania, Department of State Bureau of Occupational and Professional Affairs.
