Qualifications

• Minimum of one year of recent Home Health or Hospice experience.

Responsibilities

• Provide professional nursing care using the nursing process.
• Assess and evaluate patient status and revise the Plan of Care as needed.
• Administer medications and treatments as prescribed.
• Initiate and apply preventive, therapeutic, and rehabilitative nursing procedures.
• Monitor patient condition and report changes to physician.
• Assist with activities of daily living and promote independence.
• Educate patient and family on self-care techniques.
• Maintain a safe patient environment.
