Job summary

Sharecare is seeking a qualified RN to provide telephonic support within the care enhancement program. You will coordinate with the Primary Care Provider and disease management team to manage total healthcare, identify at-risk participants, and intervene according to program guidelines.

Responsibilities include initiating interventions, supporting educational needs, and ensuring cost-effective, quality outcomes. Yearly competency tests are required for all clinicians.

Qualifications

• Current RN license appropriate for the state where the Business Unit is located.
• High school diploma plus a nursing credential; BSN preferred.
• Minimum of 3 years of recent clinical experience.
• COVID-19 vaccine required by date of hire.

Responsibilities

• Reviews status of participants' preventive health exams per guidelines.
• Initiates interventions and conducts telephone outreach for assessment and goal setting.
• Coordinates care with Primary Care Provider and disease management team.
• Develops and facilitates care plans addressing total healthcare needs.
• Ensures continuity of care and removes barriers to access.
