Qualifications

• DDS/DMD degree required.
• Active dental license in Wisconsin.
• NPI required.
• CPR certification.
• Certificate of Insurance (COI) required.

Responsibilities

• Provide essential dental care to Reserve Service Members.
• Collaborate with DOCS Health and QTC/Leidos for referrals and scheduling.
• Maintain high standards of infection control and patient safety.
