Responsibilities

• Assess patients' physical, psychosocial, cultural and spiritual needs and report to the provider.
• Perform clinical tasks, take vital signs, and manage follow-up care to ensure efficiency.
• Collaborate with the care team to develop and monitor patient care plans.
• Educate patients and families, assess health literacy, and evaluate teaching effectiveness.
• Encourage patient engagement using motivational interviewing and self-management tools.
• Precept and mentor new staff or MA students as needed.
