Responsibilities

• Perform rheumatology-focused assessments and administer medications/injections.
• Coordinate diagnostics, labs, imaging, referrals, and follow-up.
• Document assessments and education in the EHR per regulations.
• Educate patients and caregivers on medications and lifestyle strategies.
• Collaborate with providers and teams to monitor treatment and transitions.
• Participate in quality improvement and required training.
