Job summary

Concilio is seeking an Outcomes Specialist to promote safe, timely reunification for children in out-of-home placements. You will work with the CUA team to prioritize cases for reunification and coordinate with resources to meet court and family plan objectives.

The role requires knowledge of DHS CUA guidelines, HIPAA compliance, and strong bilingual communication (English/Spanish). Experience in child welfare and social services is preferred.

Qualifications

• Familiar with DHS CUA Practice Guidelines and updates.
• Maintain confidentiality and HIPAA compliance.
• Proficiency in English and Spanish preferred.
• Familiarity with social services, child welfare and family systems services.

Responsibilities

• Promote safe and timely reunification for children in out-of-home placements.
• Prioritize reunification work for cases under 6 months in placement.
• Conduct biweekly meetings with reunification resources and monitor referrals.
• Complete walkthrough assessments to ensure home safety and document findings.
• Prepare for court, assist CM, and track progress through permanency processes.
