Job summary

Sky Lakes Medical Center is seeking a pharmacist to provide comprehensive pharmaceutical care in a hospital setting. You will prepare, dispense, and monitor medications, ensuring patient-specific pharmacotherapy is optimized and documented accurately.

The role requires knowledge of pharmacokinetics and drug utilization review, with strong communication skills. Oregon license or reciprocity eligibility with retail experience is required, and hospital pharmacy background is preferred.

Qualifications

  • Knowledge of pharmacokinetics, DUR, and distribution systems; strong communication skills.
  • Board of Pharmacy Licensed Preceptor preferred; hospital pharmacy experience desirable.
  • Oregon Pharmacist license or reciprocity eligibility with 3 years retail experience.

Responsibilities

  • Prepare and dispense medications following Pharmacy law and hospital policies.
  • Design, monitor, and evaluate patient-specific pharmacotherapy; maintain accurate profiles.
  • Provide timely drug information responses to providers and patients.
  • Counsel patients on prescriptions and medication-use education.
  • Supervise Pharmacy technicians and coordinate with support staff.
  • Ensure continuity of pharmaceutical care across acute and ambulatory settings.
  • Participate in processes for assessing and reporting medication errors.
  • Label, package, compound, bill, and deliver medications properly.
  • Comply with accreditation, regulatory, and safety requirements.
  • Manage inventory purchasing, receiving, storage, and processing.
  • Attend Pharmacy staff meetings and maximize use of technology.

QUALIFICATIONS

Required: Knowledge of pharmacokinetics, drug utilization review, drug distribution systems; Excellent verbal and written communication skills.

Preferred: Experience in a similar pharmacy practice setting is desirable. Board of Pharmacy Licensed Preceptor. Hospital pharmacy background and experience. Immunization Certification within 18 months of hire.

Lic/Reg/Cert: Possess a current Oregon Pharmacist license or eligible for reciprocity with 3 (three) years Retail experience

TYPICAL PHYSICAL/MENTAL DEMANDS

Essential Job Functions
  • Prepare and dispense medications using appropriate techniques and following Pharmacy law and liability, both state and federal. Follow the hospital’s policies and procedures
  • Design, recommend, monitor, and evaluate patient-specific pharmacotherapy. Maintenance of accurate computerized medication profiles.
  • Provide concise, applicable, and timely responses to requests for drug information from healthcare providers and patients.
  • Provide patient counseling on prescriptions and medication-use education to patients and caregivers.
  • Supervise Pharmacy technicians and utilize pharmacy support personnel effectively.
  • Ensure continuity of pharmaceutical care to and from the acute and ambulatory patient-care settings.
  • Participate in the established process for assessing, managing, and reporting medication errors.
  • Proper labeling, packaging, compounding, billing, and delivery of medications.
  • Comply in all activities with accreditation, legal, regulatory, and safety requirements.
  • Manage time effectively to fulfill practice responsibilities.
  • Manage purchasing, receiving, storage, and processing of inventory.
  • Maintain a professional image.
  • Attends Pharmacy staff meetings
  • Maximize work efficiency through the use of computers and other technologies.
