• Assist therapy department with resident mobility and transfers.
• Report any changes in residents condition immediately to the supervisor.
• Assist in transporting residents to and from appointments with the van driver.
• Safely lift and reposition residents (wheelchair to bed, bed to chair).
• Help residents sit, stand, and walk; observe for changes and report.
• Maintain confidentiality and HIPAA compliance; follow facility policies.
