Job summary

Alden Estates of Jefferson is seeking a dedicated Restorative Nurse to lead and supervise the restorative nursing program, promoting a restorative philosophy across facility departments.

You will develop, implement, and evaluate restorative care plans, ensure documentation is accurate in the EMR, and collaborate with nursing staff on assessments and plan of care. Growth opportunities exist within the Alden organization across Wisconsin and Illinois.

Qualifications

• Must possess a current, unencumbered license to practice as a Registered Nurse in Wisconsin.
• Prefer restorative certification by an accredited school or recognized accrediting agency.
• Experience developing and supervising restorative care plans and documentation.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct restorative care services and activities.
• Set up documentation flow sheets and EMR for daily restorative nursing documentation.
• Oversee restorative nursing measures on care plans.
• Perform or oversee functional assessments on admission and quarterly; collaborate with nurses on restorative plan of care.
• Oversee ADL performance and CNA documentation on ADLs; manage incontinent programs.
• Support or lead fall risk management and restraint reduction initiatives.

Restorative Nurse

Alden Estates of Jefferson, a renowned post-acute care facility, is seeking a dedicated and compassionate Restorative Nurse to join our team! As a Restorative Nurse, you will play a vital role in developing, implementing, and supervising our restorative nursing program, promoting a restorative nursing philosophy among all facility departments, and ensuring participants achieve their highest level of function.

About the Role:

As a Restorative Nurse, you will be responsible for:

• Planning, developing, organizing, implementing, evaluating, and directing restorative care services, programs, and activities in accordance with current rules, regulations, and guidelines


• Setting up documentation flow sheets and electronic medical record systems for daily documentation of restorative nursing measures


• Overseeing all restorative nursing measures are reflected on the care plan


• Performing or overseeing functional assessments on admission and quarterly as indicated, consulting with nurses in making recommendations for restorative plan of care, and working with nurses in developing restorative plan of care


• Overseeing ADL performance on units, overseeing C.N.A.'s documentation on ADL's, and overseeing incontinent management programs


• Actively participating or overseeing physical restraint reduction programs and fall management programs
