Responsibilities

  • Coordinate and monitor restorative programs for long-term and short-term patients in the skilled nursing facility.
  • Meet with nursing leaders and Director of Rehab to review residents in restorative programs.
  • Monitor medical records to ensure restorative programs are carried out and coded in MDS.
  • Assist in training CNA staff on delivering nursing restorative plans.
  • Collaborate with rehab professionals to transition residents between programs and avoid redundancy.
  • Identify and train weekend CNAs to carry out nursing restorative programs.
  • Carry out restorative nursing programs for appropriate number of patients as determined by staff.
  • Provide superior customer service to all staff and residents and maintain professional image.
  • Adhere to confidentiality standards and respect rights of patients and staff.
  • Promote Infinity Rehab's mission and values and attend in-services as directed.

Salary Range: $20.00 - $24.38

Position Summary: The Restorative Coordinator provides support and assistance in the promotion of optimal patient mobility and activities of daily living under the general supervision of a licensed nurse, with shared accountability to the Director of Rehab. The RC uses adaptive, assistive and therapeutic equipment to promote and maintain optimal activity, mobility and independence. This position supports the nursing and therapy department with clinical activities and assisting in maintaining the physical rehab environment.

Reports to: Director of Rehab

Essential Duties and Responsibilities
  • Coordinate and monitor the restorative programs for long term residents and short-term patients in the skilled nursing facility.
  • Routinely meet with nursing leaders and DOR to report and review the residents on a restorative program(s); collaborate with nursing leaders to determine appropriateness of programs and track changes or updates to the programs.
  • In partnership with the nursing facility leadership, monitor medical records to ensure restorative programs are being carried out, documented appropriately, and coded into the MDS accordingly.
  • Assist in the training of certified nursing assistant staff on the delivery of nursing restorative plans. Enlist support of other nursing leaders or rehabilitation professionals when needed.
  • Collaborate with the rehabilitation professionals and DOR to make sure residents and short-term patients are transitioned between skilled rehabilitation and appropriate nursing restorative programs. Ensure there is not redundancy between restorative programs and rehabilitation plans of care within the medical record.
  • Assist in the identification and training of weekend certified nursing assistants to carry out and document the nursing restorative programs.
  • Carry out restorative nursing programs for an appropriate number of short-term patients and long-term residents, as determined by rehabilitation and nursing staff.
  • Provide superior customer service to all staff and residents, maintaining an excellent working relationship with hospital staff and projecting a positive and professional image at all times.
  • Adhere to established confidentiality standards.
  • Demonstrate and understand the importance of and respect for the rights, dignity and individuality of each patient visitor, vendor and staff in all interactions.
  • Promote the mission, ideals and values of Infinity Rehab.
  • Attend in-services and staff meetings as directed.
  • Perform other duties as assigned.
Physical Requirements and Work Environment

Physical requirements for this position are classified as medium under the Department of Labor classification. The employee must be capable of exerting 20 to 50 pounds of force occasionally (less than 1/3 of the time), and/or 10 to 20 pounds of force frequently (1/2 of the time), and or up to 10 pounds of force constantly (2/3 or more of the time) to move objects, equipment and/or residents. While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, kneel, bend, or crouch; use hands to manipulate tools, equipment, or controls; reach with hands and arms, balance, lift, and perform medium to maximum transfer assists with patients. The employee is required to have visual and hearing acuity sufficient enough to perform job tasks efficiently and safely. The employee works mostly within the therapy room or individual patient rooms.
