Responsibilities

• Provides direct patient care and coordinates care focusing on patient education, self-management, and satisfaction.
• Administers medications and procedures per established policies and guidelines.
• Collaborates with interdisciplinary team to influence care outcomes and participate in quality improvement.
• Directs nursing education, orientation, competencies, and quality improvement initiatives.
• Evaluates patients in the Community Living Center (CLC) on admission and monthly thereafter for restorative care potential.
• Develops measurable functional goals for Veterans and collaborates with OT/PT for admissions and goal setting.
• Monitors progress toward goals, adjusts therapy and activity as needed, and completes restorative MDS sections.
