Job summary

PROGRAM AND LOCATION: VOA is recruiting a full-time Case Manager for our ReStart Family Stabilization Program in Reno, Nevada. ReStart offers both clinical and non-clinical services in an effort to reduce homelessness and increase self-sufficiency for eligible individuals and families. The services include mental health, case management, outreach to the homeless community, and housing services to qualified individuals and families. Responsibilities

POSITION SUMMARY:


Coordinate and monitor services for families presenting with immediate needs through assessment of needs, service planning, linking to resources, and advocacy with the goal of stabilizing the immediate crisis. Short-term case management will be provided to ensure immediate stabilization and increase in self-sufficiency. The Case Manager is responsible for assuring quality services in a compassionate and professional manner as it relates to the specific program objectives. Work is performed in a cost-effective and services-oriented manner as it relates to the established organizational standards.


OBJECTIVES/ACTIVITIES:


A. Responsible for performing daily work to accomplish the established objectives of the program.



  • Foster an environment free from ethnic or social prejudices and/or judgment.

  • Complete incident reports as required.

  • Communicate with families, external service providers, and co-workers in a professional manner.

  • Respond to questions and concerns.

  • Provide families with a supportive environment.

  • Comprehend and adhere to ethical guidelines and confidentiality laws.

  • Communicate and comprehend oral and written instructions.

  • Report all infractions of policies to appropriate manager or director.

  • Perform other duties as assigned.


B. Responsible for case management and crisis stabilization services.



  • Complete screening and assessments with families and create a stabilization plan.

  • Determine family’s eligibility for a variety of community services.

  • Accept referrals and complete intakes.

  • Schedule regular follow-up appointments for participants.

  • Explain and link families to agency and community services.

  • Actively assist families in accessing community services as needed.

  • Provide crisis intervention.

  • Maintain a caseload of families.

  • Request appropriate releases of information as required.

  • Enter HMIS client data into Clarity database accurately and within 24 hours.

  • Attend weekly, bi-weekly and monthly Case Staffing Meetings.

  • Open client chart and ensure all the required documents are accurately filled out and completed.

  • Complete the individualized service plan with person centered driven goals.

  • Maintain adequate case notes in DAP format.

  • Review individualized service plan with family and update as progress is made.

  • Close files by guidelines outlined within company policies and procedures.

  • Advocate on behalf of the client.

  • Make CPS and APS reports when appropriate.

  • Discuss difficult situations with supervisor.

  • Serve as a community liaison between VOA and other agencies to disseminate information to other agencies.

  • Attend other agency events and meetings to collaborate with our services.

  • Respond to agency inquiries and return calls to other organizations within one working day.

  • Maintain client confidentiality.

  • Participate in Volunteers of America and outside training sessions.

  • Perform other reasonably related duties as assigned by the supervisor.


C. Responsible for training and development of new staff.



  • Assist in department orientation and training for new employees as requested to ensure quality work outcomes.

  • Identify appropriate ongoing training for both new and existing staff and report needs to management.

  • Serve as part of the development team by providing information and support for the development of quality operations.

  • Maintain an above average working knowledge of fire, safety, and health standards to assure safe work environment for clients and all personnel.


D. Responsible for self-development.



  • Continually learn and enhance technical and interpersonal skills.

  • Attend staff meetings/assigned training and complete required certifications, i.e. CPR, First Aid, etc.


Qualifications

EDUCATION AND EXPERIENCE:


This position requires a bachelor’s degree in social work or a related field. Equivalent years of experience, or a combination of education and experience will be considered. Three years’ experience working with the homeless population and/or related field is preferred. This position requires demonstrated ability in record keeping, good written and verbal communication skills, knowledge of problems unique to adults, families, minorities, people with disabilities, substance abusers and general homeless population. Valid Nevada driver's license, reliable transportation, and ability to meet the organization’s insurance carrier guidelines are required. Must pass all required background clearances in accordance with agency, County, State, and federal standards. CPR/First Aid certification is required within 90 days of employment. Annual TB testing. Completion and maintenance of all required training and health clearances.

Pay Range

USD $25.75 - USD $25.75 /Hr.
