Qualifications

• Graduate of an accredited school of nursing required.
• Knowledge of patient- and family-centered practice and the ability to collaborate with patients, families, and care partners.
• Cultural awareness and ability to work with diverse backgrounds.
• Effective communication concepts and techniques for partnership with patients and families.
• Knowledge of patient assessment and care planning.
• Education techniques for patients and care partners about condition and treatment.
• Knowledge of medication administration and prescription guidance.
• Discharge planning and follow-up care knowledge.
• Knowledge of assessment and diagnosis methods and clinical judgment.
• Knowledge of health information documentation methods.

Responsibilities

• Interacts with co-workers, patients, their family/designated care partner(s), and other staff.
• Integrates evidence-based practice, clinical expertise, and patient preferences for delivery of care.
• Partners with patients and families in identifying learning needs and provides education.
• Minimizes risk of harm by following established nursing practices and protocols.
