Job summary

ADAPT Community Network in NYC seeks a Residence Program Specialist to support residents in a residential facility. Under supervision, you will foster dignity and independence, maintain a homelike atmosphere, and participate in interdisciplinary planning.

You will provide care, assist with daily living, safety, training, incident reports, crisis intervention if needed, transportation, meals, and supervision of activities. Medication administration is conducted under AMAP supervision.

Qualifications

• Must have a valid New York State Driver's License and maintain it in good standing.
• Ability to read, write, speak and understand English.
• Good oral and written communication skills.
• Good interpersonal skills to interact with coworkers, residents and families.
• Ability to analyze problems and determine corrective measures.

Responsibilities

• Provides care and assists with personal development and independence of the people we support.
• Maintains safety and well-being at the highest level.
• Provides training in activities of daily living and self-help/social skills.
• Maintains a personalized, homelike atmosphere in the residential facility.
• Attends staff meetings and trainings.
• Prepares concise written reports (Incident Reports, Progress Notes).
• Responds to crises and medical emergencies with decisive action.
• Administers medications after completion of medication administration course (AMAP supervised).
• Provides transportation for residents to and from activities.
• Assists with meal planning and shopping.
• Participates in interdisciplinary plans and daily information exchange.
• Performs housekeeping, cooking, laundry, and light maintenance as needed.
