Fahrman Center at Lutheran Social Services of Wisconsin and Upper Michigan seeks a Substance Use Disorder Counselor to join our 42-bed residential treatment facility in Eau Claire, WI. You will provide individual and group counseling, develop personalized treatment plans, and support clients throughout their recovery journey in a collaborative, recovery-focused setting.

Qualifications

• Associate's degree in Human Services, Corrections, or related field.
• Valid Wisconsin SAC-IT credential or higher.
• Valid driver's license and reliable transportation.
• Ability to pass background, caregiver, and medical clearances.

Responsibilities

• Provide individual and group substance use counseling and case management services.
• Complete screenings, assessments, referrals, and documentation.
• Develop, implement, and monitor individualized treatment plans.
• Coordinate care with community providers and multidisciplinary team.
• Maintain accurate clinical documentation and provide family education.
