Job summary

Milestone, Inc. seeks a Residential Supervisor in Loves Park, IL. You will supervise assigned homes and direct care staff across multiple shifts, ensuring participants’ daily routines and community activities align with program plans.

You will coordinate scheduling, monitor health needs with the consulting RN, and ensure compliance with all applicable guidelines and Milestone policies. Strong communication with guardians is required.

Qualifications

• Minimum age 21.
• Prefer at least 1 year in the field.
• Prefer at least 1 year in a supervisory role.
• Must be approved to drive Milestone vehicles.
• Must be able to intervene physically in behavioral/health crises.

Job Details: Job Location: Loves Park, IL 61111, Position Type: Full Time, Education Level: High School, Salary Range: $25.50 - $29.97 Hourly, Travel Percentage: None, Job Shift: 1st Shift, 2nd Shift and 3rd Shift, BASIC FUNCTION:

Under the supervision of the Residential Services Director and/or V.P./Administrator, provides direct supervision to assigned homes. Ensures that individuals are actively participating in programs; activities on the home and in the community; and the daily routine of their group homes. Also ensures compliance with all state and federal regulatory guidelines as well as all policies and procedures of Milestone, Inc.

DESCRIPTION OF ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Reliable and prompt in attendance as scheduled, demonstrates flexibility as necessary to meet individual, home, and/or facility need.
• Supervises and assists in evaluating job performance of direct service providers, including Team Leaders. This includes disciplinary action as needed.
• Functions as a trainer for all direct care staff in orientation to specific job responsibilities and in implementation of new programs and procedures. As well as activities on the home and in the community and documentation of same.
• Assists with scheduling meetings ensuring adequate staff coverage at all times and within established FTE.
• Coordinates functioning of AM, PM, and nocturnal shifts to ensure continuity of programming for the individuals served.
• Coordinates, with input from individuals served and staff, all home-based-community activities, including development of new activities and resources.
• Functions as a liaison with other agencies and community resources.
• Responsible for compliance with all applicable guidelines for assigned homes.
• Consults with the QIDP and/or RSD in the development of individuals serve plans/personal plan implementation strategies for each assigned individual.
• Monitors completion of all required (non-programmatic) records and individual programming.
• Participates in, or facilitates, annual meetings with the interdisciplinary team.
• Monitors group home program cash and individual personal funds on a daily basis. Also consults with Milestone’s Accounting department in order to maintain accurate, up-to-date records.
• Monitors health needs of the individuals and ensures that all needs are met. Communicates regularly with consulting RN, discussing immediate and future medical needs of each individual.
• Ensures that all individual’s personal needs are recognized, met and documented including clothing, personal hygiene, and personal possessions.
• Ensures that all necessary medical/dental exams are scheduled and any resulting orders or treatments are carried out. Also maintains appropriate medication records.
• Maintains and encourages regular contact with guardians and families, ensuring they are well informed on all progress and occurrences involving their individual.
• Ensures assigned homes are maintained in a clean, order, and safe manner.
• In compliance with the Milestone, Inc. philosophy and policies toward human rights, immediately reports any questions or concerns to the Residential VP/Administrator, the Human Rights Committee, and/or the Governing Agency (OIG/Dept Public Health).
• Develops, in consultation with the IDT, QIDP, and RSD, all behavioral programs for individuals.
• Monitors progress through documentation and observation.
• Functions as a member for all assigned homes in development of new programming and in completion of special projects.
• Functions as supervisor on duty/on-call supervisor to handle unusual and emergency situations, answer questions, offer support, and/or assist with scheduling issues/changes.
• Monitors grocery charges to ensure the assigned homes stays within the assigned budget on a monthly basis.
• Submits monthly paperwork to Residential VP/Administrator on time, including: scheduled medical appointments, annual physical and dental appointments vehicle inspections, board reports, antibiotic use, maintenance walk-throughs, grocery shopping charges, copy of home schedule evacuation drills completed, and monthly outing summary.
• Must be able to respond promptly to emergency situations.
• Will adjust schedule as needed to meet the needs of the individuals served.
• Performs other duties as directed or assigned.
