Job summary

Centerstone in Perryville, MO is seeking a Residential Home Coordinator to lead daily operations of residential homes, ensure safety and wellbeing, and drive person-centered care for individuals served.

You will mentor Direct Support Professionals, coordinate schedules, participate in planning meetings, and collaborate with healthcare teams to achieve individual goals while maintaining compliance and high-quality services.

Qualifications

• High school diploma or equivalent required.
• One year of experience in intellectual/developmental disabilities field required; or MO Quality Outcomes training.
• Able to read and interpret safety rules, instructions and procedures.

Key Responsibilities:

• Lead the day-to-day operations of residential homes while fostering a positive, person-centered environment for individuals served.
• Support, coach, and provide incidental teaching for Direct Support Professionals to promote quality care and professional growth.
• Coordinate and maintain daily activity schedules that encourage engagement, independence, and achievement of individual goals.
• Advocate for individual rights and actively participate in Individual Support Plan meetings, RN visits, and person-centered planning activities.
• Train staff regarding individuals' medical needs, preferences, medications, and potential side effects to ensure quality support and compliance.
• Accompany individuals to medical appointments and assist with management of client finances as appropriate.
• Collaborate with Program Coordinators and healthcare professionals regarding monthly RN visits, special diets, adaptive equipment training, and quality improvement initiatives.
• Monitor residential documentation, including daily notes, goal progress, health tracking, and authorized service records to ensure accuracy and timeliness.
• Conduct audits and review Medication Administration Records (MARs) to maintain compliance with licensing and certification requirements.
• Inspect homes regularly to ensure health, safety, cleanliness, and maintenance standards are consistently met.
• Maintain safety kits, coordinate safety drills, and ensure required training and documentation are completed.
• Complete incident and event reports and address follow-up actions as needed.
• Coordinate staffing coverage during absences, cover open shifts when necessary, and respond to program needs on a 24/7 on-call basis.
• Monitor mileage usage and other assigned resources within budget guidelines.
• Participate in organizational committees and perform other duties as assigned.

Education, Experience, and/or Credential Qualifications:

• High school diploma or equivalent required.
• One year of experience working in the field of intellectual and/or developmental disabilities required; OR successful completion of training in Missouri Quality Outcomes approved by the Division of Developmental Disabilities regional office.
• Proficient in reading and interpreting safety rules, operating instructions, and procedures.
• Ability to write routine reports and correspondence accurately and professionally.
• Strong verbal and written communication skills with the ability to interact effectively with individuals served, families, employees, and community partners using person-centered language.
• Ability to follow written, verbal, and diagram-based instructions.
• Effective problem-solving skills and ability to make sound decisions in a variety of situations.
• Competency in basic mathematical calculations involving whole numbers, fractions, and decimals.
• Strong reading comprehension and documentation skills.
• Reliable means of communication.

• Must be at least 18 years of age.
• Current valid driver's license required.
• Acceptable driving record required.
• Current automobile insurance required.
• Successful completion of background screenings, including criminal record, driving record, and abuse/neglect checks required.
• Ability to lead teams, communicate organizational goals, and promote employee engagement.
• Ability to effectively manage change, resolve employee concerns, and maintain professionalism and confidentiality.
• Commitment to protecting confidential and Protected Health Information (PHI) in accordance with HIPAA and company policies.

Physical Requirements

• Ability to sit or stand for extended periods of approximately 2 to 4 hours at a time.
• Ability to walk for short periods while conducting field-based or residential activities.
• Frequent use of hands and fingers for writing, typing, filing, operating office equipment, and handling documentation.
• Ability to occasionally lift and/or move up to 50 pounds.
• Ability to perform the essential functions of the position with or without reasonable accommodation.
