Job summary

Easterseals NH is seeking a Family Outreach Worker in Manchester, NH, for our Residential Educational Services program. You will provide case management to youth in the residential program and to their families, collaborating with social services and the clinical team to support treatment goals.

You will coordinate with families, ensure medical appointments are up to date, and maintain compliant documentation while communicating with collaterals and partners in the community.

Qualifications

• A minimum of a Bachelor’s Degree in Human Services or related field of study.
• Two years of experience working with children and families.
• A valid driver’s license, reliable transportation, Auto Insurance.

Responsibilities

• Provide case management services to youth in the residential program and to families in the greater Manchester area.
• Collaborate with social services to apply family-centered practices and community-based work.
• Assess family resources and help youth and families identify strategies to support treatment goals.
• Coordinate with residential and clinical teams to support clients’ treatment plans.
• Ensure all medical appointments are up to date and maintain documentation including authorizations and reporting.
• Maintain clear communication with families and collaterals.
