Community Health Resources is seeking a Residential Aide for a full-time night shift at Milestone in Putnam, CT. The role supports women with co-occurring disorders through counseling, case management, and aftercare planning within a trauma-informed program.

Responsibilities include monitoring resident medications, facilitating groups, coordinating care with staff and families, and maintaining documentation in line with CHR standards. Compensation is provided per-hour with comprehensive benefits.

Qualifications

• Education: High School Diploma or equivalent.
• Experience: Experience in Human Services.
• Licensure/Certification/Registration: Valid driver's license and CPR & First Aid; certification required or obtained within 90 days.

• Monitor medications for all residents per policy and procedure.
• Communicate day-to-day events to staff using appropriate methods.
• Teach and assist residents in rehab areas including nutrition, hygiene, budgeting, and home care skills.

• Collaborate with providers and families to coordinate care.
• Assist residents with medical and psychiatric needs and maintain documentation.

ABOUT THE PROGRAM

Milestone is a 30-day intensive level 3.7 residential treatment center designed to accommodate and treat women with a history of substance abuse, trauma, and mental health issues. Our program is recovery-oriented, gender-responsive, and trauma-informed. The overall goal of treatment is to help women find real hope for recovery. Milestone offers individual and group counseling, case management, and aftercare planning services. This program also provides treatment for co-occurring disorders, with substance abuse as their primary diagnosis. Treatment is individualized and targeted around the needs essential to moving forward toward employment, housing, and family stability. Milestone is part of CHR's Center of Excellence for Women/Women and Children at our Putnam campus. Length of day is less than 30 days and are then referred out to a step-down program to further address their co-occurring disorders.

ABOUT THE POSITION

Duties & Responsibilities

• Maintains physical environment and program standards to CHR, DMHAS, JCAHO and any other licensing standard
• Monitors medications of all residents in accordance with the Adult residential medication policy and procedure.
• Effectively utilizes appropriate communication methods/tools to transfer information to other staff regarding the day’s events
• Teaches and assists all residents in areas of rehab including: nutrition and healthy living, meal preparation, hygiene, laundry, banking and budgeting, and basic home care skills.
• Assists/Manages administrative functions and tasks at the direction of the supervisor
• Prepares and facilitates educational and skill building groups and one to one interventions as directed by the treatment plan.
• Collaborates and coordinate care with providers and families as needed.
• Assists residents in addressing medical and psychiatric needs.
• Maintains documentation in compliance with agency and program standards.
• Participates in staff meetings, supervision, trainings and other meetings as assigned.
• Additional duties as required.

QUALIFICATIONS

Education: High School Diploma or equivalent.

Experience: Experience in Human Services with responsible work history.

Licensure/Certification/Registration: Valid driver’s license required with good driving record. CPR & First Aid Certification required OR must obtain within 90 days of employment.

COMPENSATION*

$19.285/hour min-$27.9863/hour max

• Actual rates within this range are determined at the time of offer and are based on relevant experience, as well as adherence to our internal equity policy. (Please note, for part-time positions this range would be pro-rated based on hours worked)
