Job summary

Community Health Resources, Inc. (CHR) is seeking a Residential Aide for the Milestone program in Putnam, CT.

This full-time on-site role offers Friday-Tuesday shifts, 12am-8:30am, with 40 hours per week and a focus on recovery-oriented care for women with co-occurring disorders. Responsibilities include supporting daily living activities, monitoring medications, documenting and coordinating with treatment teams, and facilitating groups under the treatment plan.

Qualifications

  • Education requirement is a high school diploma or equivalent.
  • Experience in Human Services with responsible work history.
  • Valid driver’s license with good driving record; CPR & First Aid certification required or within 90 days.

Responsibilities

  • Maintains physical environment and program standards to CHR, DMHAS, JCAHO and licensing standards.
  • Monitors medications of residents per policy and procedure.
  • Communicates day’s events to staff using appropriate methods/tools.
  • Teaches residents in rehab areas: nutrition, budgeting, hygiene, and basic home care skills.
  • Assists/administers tasks as directed by supervisor.
  • Prepares and facilitates educational and skill-building groups and one-to-one interventions.
  • Coordinates care with providers and families as needed.
  • Addresses residents' medical and psychiatric needs as required.
  • Maintains documentation in compliance with agency standards.
  • Participates in staff meetings and trainings; performs additional duties as required.

JOB TITLE: Residential Aide

EMPLOYMENT TYPE: Full-time, 40 hours/wk

SCHEDULED HOURS: Friday-Tuesday, 12am-8:30am

ABOUT THE PROGRAM: Milestone

Milestone is a 7-30-day intensive level 3.7 residential treatment center designed to accommodate and treat women with a history of substance abuse, trauma, and mental health issues. Our program is recovery-oriented, gender-responsive, and trauma-informed. The overall goal of treatment is to help women find real hope for recovery. Milestone offers individual and group counseling, case management, and aftercare planning services. This program also provides treatment for co-occurring disorders, with substance abuse as their primary diagnosis. Treatment is individualized and targeted around the needs essential to moving forward toward employment, housing, and family stability. Milestone is part of CHR's Center of Excellence for Women/Women and Children at our Putnam campus. Length of day is less than 30 days and are then referred out to a step-down program to further address their co-occurring disorders.

ABOUT THE POSITION: Residential Aide

Duties & Responsibilities

  • Maintains physical environment and program standards to CHR, DMHAS, JCAHO and any other licensing standard
  • Monitors medications of all residents in accordance with the Adult residential medication policy and procedure.
  • Effectively utilizes appropriate communication methods/tools to transfer information to other staff regarding the day’s events
  • Teaches and assists all residents in areas of rehab including: nutrition and healthy living, meal preparation, hygiene, laundry, banking and budgeting, and basic home care skills.
  • Assists/Manages administrative functions and tasks at the direction of the supervisor
  • Prepares and facilitates educational and skill building groups and one to one interventions as directed by the treatment plan.
  • Collaborates and coordinate care with providers and families as needed.
  • Assists residents in addressing medical and psychiatric needs.
  • Maintains documentation in compliance with agency and program standards.
  • Participates in staff meetings, supervision, trainings and other meetings as assigned.
  • Additional duties as required.

QUALIFICATIONS:

Education: High School Diploma or equivalent.

Experience: Experience in Human Services with responsible work history.

Licensure/Certification/Registration: Valid driver’s license required with good driving record. CPR & First Aid Certification required OR must obtain within 90 days of employment.

COMPENSATION*:

$18.3818/hour minimum, and up commensurate upon experience

*Actual rates are determined at the time of offer and are based off of relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
